HIPAA Notice of Privacy Practices
This notice explains how Baron II Drug & Surgical may use and disclose your protected health information and how you can get access to this information.
As part of the federal Health Insurance Portability and Accountability Act of 1996 HIPAA, Baron II Drug & Surgical has created this Notice of Privacy Practices. This notice describes our privacy duties and your rights regarding your protected health information PHI. PHI is information that identifies you and relates to your past present or future physical or mental health or condition and related health care services. We must protect the privacy of PHI that we create or receive. We will follow the terms of this notice. For any uses or disclosures not described here, we will obtain your written authorization, which you may revoke in writing at any time. We may change our privacy practices and this notice as the law allows. Updated notices will be posted in the pharmacy and available upon request. HOW WE MAY USE AND DISCLOSE YOUR PHI Treatment We use and share PHI to fill prescriptions, counsel you, and coordinate your care with your prescribers. Payment We use and share PHI to bill you or your health plan, verify benefits, and obtain prior authorizations. Health Care Operations We use PHI to run our pharmacy, improve services, train staff, and perform quality checks. ADDITIONAL USES AND DISCLOSURES WITHOUT AUTHORIZATION As required by law Public health activities including adverse event reporting and product recalls Victims of abuse neglect or domestic violence as allowed by law Health oversight activities such as inspections and audits Judicial and administrative proceedings with a valid order Law enforcement when required Deceased individuals and coroners or medical examiners Organ eye or tissue donation Research with proper approval or your authorization Averting a serious threat to health or safety Specialized government functions Workers compensation Disaster relief and to notify family or caregivers Business associates who must safeguard your PHI OTHER CONTACTS We may contact you for refill reminders, treatment alternatives, or health related services. You may opt out of fundraising messages if any are sent. ALL OTHER USES AND DISCLOSURES We will obtain your written authorization for any other use or disclosure of PHI. You may revoke that authorization in writing at any time. YOUR HEALTH INFORMATION RIGHTS 1. Request restrictions on certain uses and disclosures. We are not required to agree to all requests. 2. Request confidential communications by alternative means or locations. 3. Inspect and obtain a copy of your PHI. Reasonable fees may apply as allowed by law. 4. Request an amendment of your PHI if you believe it is incorrect or incomplete. 5. Receive an accounting of certain disclosures of your PHI. 6. Receive a paper copy of this notice upon request. REVISIONS TO THIS NOTICE We may change this notice and make the new notice effective for all PHI we maintain. The revised notice will be available at our pharmacy and on our website. COMPLAINTS If you believe your privacy rights have been violated, you may file a complaint with Baron II Drug & Surgical or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
